## Summary
German cancer documentation must code each tumor diagnosis in both ICD-10-GM and ICD-O-3. Small open-weight LLMs suit this work because they can run locally and protect patient privacy, but they code German diagnoses poorly. Lenz et al. test whether instruction-tuning on freely available German catalogues closes this gap. They converted Alpha-ID (the ICD-10-GM index), ICD-O-3 and the OPS procedure classification into more than half a million short German question-answer pairs. These cover code lookup and yes/no recognition of tumor diagnoses, with non-tumor diagnoses and procedures used as negatives. Eight Qwen, Llama and Mistral models (7B to 70B) were fine-tuned. They were then tested on real, deduplicated diagnosis strings from the Mainz tumor documentation system. Two reviewers checked a sample to estimate how often the short texts actually contain enough information to derive the gold code. Fine-tuning raised exact ICD-10 accuracy from near zero to roughly half of cases, close to that estimated ceiling. It also eliminated malformed outputs and narrowed the gap between small and large models. The paper shows that public catalogues are an underused source of training data.

## Key points
- **Training data (ICDOPS-QA-2024).** The dataset has 518,116 QA pairs built from:
  - Alpha-ID 2024: 13,600 tumor descriptions mapped to 809 codes.
  - ICD-O-3: 400 topography codes and 1,145 morphology codes.
  - OPS 2024: procedures used as negatives.
  - Each question type is phrased in 4–10 equivalent wordings. The dataset and the best checkpoints are released on Hugging Face.
- **Test set.** It contains 2,024 unique diagnosis texts from 2023–2024. A 100-case sample was double-annotated and put the achievable ICD-10 accuracy at 60–79% for exact codes and 81–94% at the three-character level (95% CI).
- **ICD-10 results.**
  - Exact accuracy rose from 1.4–24% to 41–58%.
  - Partial accuracy rose from 31–74% to 73–83%.
  - Malformed codes fell to 0%.
  - Tumor-diagnosis recognition reached 99%.
- **ICD-O topography results.** Topography stayed harder, at 22–40% exact and 56–67% partial after tuning.
- **Model size and reasoning.**
  - Larger models scored better both before and after tuning, but the gap shrank after tuning.
  - Qwen3's reasoning mode generally scored lower than fine-tuning and was more than 100× slower.

## Interesting ideas
- Turning a reference catalogue into many paraphrased question templates gives cheap, broad supervision without any annotated corpus, and the paraphrases teach robustness to prompt wording.
- Pulling negatives from a neighbouring but conceptually different catalogue (procedures vs. diagnoses) teaches the model where a concept's boundary lies, not just what lies inside it.
- Before judging a model, the authors had two annotators estimate how often the input actually determines the gold label, so scores can be read against a realistic ceiling.
- Training on deliberately short, code-only answers drove malformed outputs to zero and made outputs fast to generate and trivial to parse.
- The authors suggest a cost-aware hybrid, with a lightweight model for easy cases and an LLM only for hard ones, especially since slow reasoning modes did not pay off here.

## For Mova

How these ideas could serve Mova, our small open language model — its files, stages and tests:

- **I1** (learn): VerbNet classes, WordNet senses and FrameNet frames could be rendered as varied QA pairs to train or probe the LLM teacher tiers on Mova's own label inventory.
- **I2** (world): ontology typing could be trained with near-miss negatives, e.g. events vs. states or artefacts vs. organisms, so entity classification learns real distinctions.
- **I3** (answer): for fable QA and school tests, mark whether each answer is derivable from the text itself, so world-knowledge gaps are separated from reader errors.
- **I4** (learn): LLM teacher prompts could require terse, schema-constrained labels (a deprel, a frame name) so silver data needs no fragile output parsing.
- **I5** (tree): let the perceptron parser handle sentences where witness parsers agree, and escalate only disputed ones to gparse or an LLM teacher.
