## Summary
Even non-experts can now rapidly build prototypes of healthcare AI, including mental health chatbots, wellness-tracking apps and stress-monitoring systems. They can still cause harms like bias, privacy loss or unequal access. Existing aids that flag risks automatically, such as RiskRAG, AHA! and Farsight, may leave people less involved in ethical reasoning.

The authors propose a human-centered foresight framework. It turns a description of an AI application into plausible near-future user stories and pairs them with multi-agent red-team discussions. A three-layer agent pipeline works as follows:
- it first builds the scenario's people and environment;
- it then simulates their interactions with the system, with tool outputs produced at runtime;
- it finally rewrites the simulation logs as short stories.

In a user study, participants exposed to stories and discussion spread the harms they identified across all 17 harm categories. Participants without stories concentrated mainly on privacy and well-being. The results suggest that narrative framing helps people foresee risks and benefits for diverse users before deployment, keeping human judgment central instead of handing speculation over to AI.

## Key points
- The source set contains 38 consumer-health AI applications, collected manually from Wired articles, industry product descriptions and PubMed papers. Each was mapped to a realistic use-case scenario.
- The pipeline has Context, Logic and Narrative layers. Its agents include Persona, Environment Modeling, Incentive/Constraint and Storyteller agents.
- In the STORY+DISCUSSION condition, harms were distributed more evenly over 17 harm types. In the no-story condition, 79.1% of responses focused on privacy and well-being.
- Evaluation used model cards as a tool and a single LLM judge for pairwise comparisons, checked against human agreement. The user study was small and involved mostly technically trained university participants.
- Code, prompts and the dataset are released at github.com/Language-Technology-Lab/speculative-storytelling-healthcare. The work was funded by NSF Grant No. 2145357.
